Transcript Requests

The transcript is the official record of course work and grades, per term, for each school attended within the University of Pennsylvania. It also displays degrees awarded, honors earned, academic notes and your G.P.A. (if applicable).

How to Order

On the Web

Graduates of the University of Pennsylvania School of Medicine can visit the Student Portal and scroll to "Transcript/Dean's Letter Request Form". There you can print a request form to send up to four copies of your transcript. There is a $3 fee per request for graduates (no fee for current School of Medicine students).

Information Required for a Transcript Request

• Name (as it appears on your student record)

• Graduation Year
  

• Type of transcript that is being requested:

  • Official - Arrives in a signed, sealed envelope and contains completed course work

  • Unofficial - Arrives traditionally (not in a signed, sealed envelope) and contains completed course work as well as course work in progress

• Number of transcripts

• Addresses where the transcripts are to be sent

• Daytime phone number, e-mail, or pager number where you can be contacted if any problems are encountered processing your order

• Your (the student's) signature to release the transcript
