Phase 1

  • Interviews with key opinion leaders, focus groups, observations, surveys, concept mapping

Phase 2

  • Vital statistics, state and national health surveys, medical and administrative records

Phase 3

  • Assessments of knowledge, attitudes, beliefs, preferences, existing skills, and self efficacy

Phase 4

  • Intervention matching, mapping, pooling, and patching

Phases 5-8

  • Process Evaluation: Interviews or surveys with practitioners to determine whether program was implemented according to protocol
  • Impact Evaluation: Surveys to assess change in predisposing, reinforcing, and enabling factors
  • Outcome Evaluation: Post-program assessment of outcome indicators
