- Send the following paperwork to SOM HR (via interoffice mail or hand-deliver to 118 Blockley Hall, c/o Kathy
Rick):
- Completed copy of http://www.upenn.edu/affirm-action/online_AACF.pdf form
- Completed employment applications of all applicants interviewed for position
- Resumes of all applicants interviewed for position
- Two copies of the selected applicant's resume
- Signed and completed employment application for selected applicant (internal and external)
- Two reference checks (they MUST be from the last 2 supervisors)
- Draft of offer letter (offer letters can be found in Recruitment Forms)
- Salary/Start
Date Authorization Request Form - This is simply a memo stating:
- Title
- Reference Number of Position
- Name of Final Candidate
- Requested Salary and Start Date
- Who in your Department for HR to call to
discuss Salary.
- Once SOM HR has received all of the paperwork they will contact you to let you know if they have authorized the salary
request. SOM Recruiter will then sign and complete the HR 1 / HR 2 form
and notify your department when it will be available for pickup at 118
Blockley Hall.
- Immediately after the salary has been authorized,
your department can send the candidate the offer letter.

- After candidate has signed the offer letter,
have them fill out:
- Online I9
- W-4
- Voluntary Self-Identification Form
