Web-Based Materials Access Agreement

POOL COOL is a research project that was conducted at the University of Hawaii, Emory
University and The National Recreation and Park Association (NRPA)
 with grants from the National Cancer Institute (NCI) and the Centers for Disease Control and Prevention (CDC).

DESCRIPTION
POOL COOL is a sun-safety education program for use at swimming pools. Its main objective is to
increase awareness, motivation, and sun protection practices among children ages 5-10 who take
swimming lessons, their parents, aquatics staff, and other pool users.

WHY THIS PROGRAM IS IMPORTANT
Skin cancer is the fastest growing form of cancer in the United States, with one million new cases
diagnosed each year. It's widely known that protection from the sun's rays could prevent about 90%
of all skin cancer cases. Youth are the most at risk for overexposure to the sun's rays, and those
who receive the most exposure before the age of 18 tend to have the highest rates of skin cancer
later in life. Protection includes covering up, seeking shade, using sunscreen properly, and avoiding
staying outside too long during peak sun hours.

POOL COOL OFFERS FREE MATERIALS
We are eager to offer the Pool Cool Program information to any pool site that may be interested,
provided that the information is not sold or further copied and distributed without our knowledge
and consent. Each pool is free to conduct as much or as little of the program as desired.

WHAT WE ASK FROM YOU
We would like to be able to contact you at the end of the summer (and possibly future summers) to
ask you about how your pool used the program materials - how much of it you used, how well you
liked it, etc. We will send you a short survey with a business reply envelope for you to return it in,
or you can complete a form on-line if you prefer. It will only take a few minutes to complete.

AGREEMENT OF TERMS
I have read and understand the above information, and I am willing to be contacted and to fill out
a survey at a later time as designated by the Rollins School of Public Health (RSPH). I further
agree not to sell or otherwise distribute the information I receive without permission of RSPH. I
also agree not to hold RSPH liable for my use or misuse of the materials provided.
