Important: PSOM COVID-19 Vaccine Instructions

In follow-up to our note from last week regarding vaccines, the Perelman School of Medicine is working with Penn Medicine to offer COVID-19 vaccines to our PSOM community beginning on April 13, 14, and 15.
This opportunity is in addition to the University’s recently announced Penn Wellness vaccine clinic.
To indicate that you would like to receive a vaccine through any offered Penn vaccine clinic, please fill out the PSOM Vaccine Interest Form by Sunday, April 4, 2021. Please do not share this link with others.
We encourage you to submit both the University and Penn Medicine surveys, as well as signing-up via local lists, to maximize all potential opportunities for vaccination. Additional dates may be established based on the number of responses to this survey.

Please note that completing the interest form does not mean you are registered for the vaccine clinic. If you fill out the PSOM Vaccine Interest Form at the link above, you will receive an email the week of April 5th with important information, including how to schedule your vaccine appointment through the myPennMedicine patient portal. The email will come from “Penn Medicine” from the email address “PennMedicineCOVIDinfo@pennmedicine.upenn.edu” and include the subject line “Scheduling Your COVID-19 Vaccine.” Be on the lookout for this email the week of April 5th.
The PSOM vaccine clinic on April 13, 14 and 15 will be on the 15th floor of the Perelman Center for Advanced Medicine (PCAM). Second doses will be scheduled onsite when you receive your first dose and will be administered on May 4, 5, and 6.
