Increasing Access and Improving Outcomes

The Center serves as our academic hub for groundbreaking research and community engagement to accelerate health care transformation and lead national efforts to eliminate disparities.

The Center for Community and Population Health (CCPH) focuses on research and community initiatives to reduce health disparities through improved access to care and collaborative partnerships. Examples of initiatives housed within CCPH include the National Center for Integrated Behavioral Health, Cancer Screening Outreach and Navigation, and our Community Clinic / Provider partnerships.

Community Clinics & Partners

For families and individuals who live in areas of the city where access to medical services is limited — and in many cases, nonexistent — a trip to the doctor can mean traveling across the city for basic care. To help fill this void,  we bring support and care to the communities needing it most, with a major focus on partnering with organizations in urban Philadelphia neighborhoods. Our attending and resident physicians are an integral part of the care teams at several Federally Qualified Health Centers, and our students serve the community at student-led clinics.

Research Initiatives

The Department of Family Medicine and Community Health has ongoing research in areas of cancer screening and prevention, behavioral health, maternal child health, social determinants of health, chronic disease, Asian and cultural medicine, trauma informed care, intimate partner violence, as well as educational research to improve the training of physicians.Our faculty includes experts in patient centered outcomes, mixed methods, qualitative, and community based participatory research methodologies.

Global and Population Health Fellowships

We prepare primary care and community health leaders of the future with inter-professional educational opportunities to help extend the reach of Penn’s programs. Our fellowship programs train leaders in global health, population health, sports medicine, and research.
