Health System Data Requests including QI:
For health system data requests including QI, make sure that you are on the UPHS network (you must be on the UPHS network to access the ticketing system), click this link PennDnA Report or Data & Analytics Request and complete the online form in its entirety. You will receive a request number for tracking purposes. Based on scope and complexity, your request may be routed to Entity Executive Leadership for review and approval before it is assigned to a team member.
Research Data Requests:
Please use the following link to submit a research data request via the Penn Medicine intake form. Requests will be evaluated internally by a team comprised of PSOM staff from cores within OCR (Office of Clinical Research), IBI (Institute for Biomedical Informatics), CCEB (Center for Clinical Epidemiology and Biostatistics), & PMBB (Penn Medicine BioBank) to determine (i) scope and complexity of work, (ii) available funding, (iii) whether requests can be satisfied by investigators using self-service tools, and (iv) allocations of resources.
