Community Initatives
The Department of Family Medicine and Community Health has expanded their school-based health programming by establishing a twice-weekly health and wellness clinic at West Philadelphia High School.
Community Partnerships
Community Partnerships for Wellness (CP4W) Community Partnerships for Wellness improves health equity in the West Philadelphia community through school-based partnerships that deliver primary care services. The Community Partnerships for Wellness hosts twice weekly clinics at West Philadelphia High School, regular clinics with community partner organizations (such as ACHIEVEability and Mighty Writers), and seasonal pre-participation sports physical clinics in schools across West and Southwest Philadelphia in partnership with the Division of Sports Medicine. We provide acute care such as blood pressure testing, A1C, flu and COVID vaccines, and physicals. We also provide supported navigation to specialty care, behavioral health care, and other social health and wellness services. The department is continuing to expand its Community Partnerships for Wellness.
