About the Program

Heart disease is the leading cause of illness and death in the United States. But we can change this together. Penn Medicine Healthy Heart is a six-month texting program to help you improve your heart health from home. We're excited that you're here to learn more about easy steps you can take today.

Healthy Heart can help you to lower blood pressure and cholesterol, eat healthier, and reduce smoking.

Who Qualifies?

Penn Medicine Healthy Heart is currently available to patients at select Penn Medicine primary care practices.  Eligible patients meet the following criteria:

Eligibility criteria for Healthy Heart are ages 35-80 with high blood pressure and high under or untreated cholesterol and either high risk of cardiovascular disease, diagnosis of cardiovascular disease, or diagnosis of diabetes

Stay Engaged

Each month, we will text to remind you to check your blood pressure and text us the result. Based on your numbers, we will help you create and stick to a plan for blood pressure and cholesterol medicine and lifestyle changes that are right for you. If you have questions, we can text, speak on the phone, or even schedule a video visit! We will update your primary care provider on your progress.

Graduate

We’ll text you for one final BP check after six months. We’ll also help you do a free cholesterol test at a lab near you. The results will help you know how well the program worked for you. It will also help us improve the program for future patients.
