Simple Web Forms via MachForm
What is MachForm?
MachForm is a web application that can be used to create simple online forms. Submissions can later be viewed online or imported into Excel by designated MachForm administrators.
While MachForm can be used to create surveys, there is another, better tool for this purpose called Qualtrics. The Perelman School of Medicine has a site license and full support is provided by the vendor.
Advantages & Capabilities of MachForm
MachForm provides you with a simple way to gather data of any scope. Within the user-friendly interface you have the ability to update, duplicate and delete forms, and view, manage, export, sort and delete entries.
Please note that MachForm should NOT be used to collect sensitive information, such as confidential patient or research study participant information. Please use REDCap for this purpose. Collection of HIPAA data via Machform is NOT permitted.
Types of MachForm Access
As a MachForm editor you may have one or all of the following privileges:
- Edit forms
- Create forms
- Edit entries (submissions)
- View/download entries
