Health Information for Entering Students

Health History and Immunization Forms

All entering students must meet the University's immunization and health history documentation requirements. The documentation forms are mailed to your home address as listed on AMCAS. Details for Health Care Professional (HCP) Students can be found by going to Immunization Requirements | Wellness at Penn (upenn.edu).  Immunization Form for PSOM student: Immunization form here.  Immunizations records should be submitted to the University as close to July 1 as possible. Records should be complete and compliant.

Health Insurance Information

• All students are required to have health insurance.
• Please review the information regarding the Penn Student Insurance Plan (PSIP) and criteria for alternate insurance on the website (Insurance Requirements | Wellness at Penn (upenn.edu)).
• Students must enroll in PSIP or submit a request for a waiver on-line between July 1 and August 31, 2025.
• If you do choose the PSIP, coverage will start on August 1.
• Failure to enroll or submit acceptable alternative coverage will result in you automatically being enrolled in the PSIP.
• For those who enroll in the PSIP, the annual premium is billed to your student account in 2 increments. The first is billed in August and the second is billed in December.
