Instructions for Completing the Cadaver and Body Parts Request Form ("CBP Request")
Below you will find a step by step guide and helpful definitions to assist you in filling out the CBP request form. You are strongly encouraged to reference the University Policy "Use of Cadavers and Recognizable Body Parts for Educational and Research Purposes at the University of Pennsylvania" (Effective Date: April 1, 2019) for more information. Project Title
The title used for the project under which the CBP Request is being submitted. Once the request has been approved, the project title will be used to identify an Approved Project, which is the use of cadavers/parts for educational and research activities that has received formal written approval by the Operational Committee, as indicated on a Cadavers and Body Parts Request form.
Project Leader
The Project Leader must be a faculty member of the University of Pennsylvania, is responsible for the actual use of cadaver/parts during an Approved Project and assumes a lead role in directing and carrying such activities as described in the CBP Request. (Note: The Operational Committee ("OC") may grant an exception to the requirement that a Project Leader must be a faculty member provided that there is appropriate departmental oversight and that the department's chair or designee has approved the appointment of the Project Leader.) The Project Leader's responsibilities may not be delegated to a student or post-doctoral trainee. The Project Leader:
- Submits the CBP Request to the OC and commences use of cadaver/parts only after receipt of a CBP Request approved in writing by the OC.
- Maintains records pursuant to Section VI. K. of the policy.
- Responsibilities listed in Section V.F. also apply to the Project Leader, unless assumed by the Anatomical Specimens Coordinator.
Department
The sponsoring department for the project; namely, the home department of the Project Leader.
Departmental Budget Code
The 26-digit code used by departments to bill for costs associated with the Approved Project.
Phone/Address/Email
Project Leader's contact information. Contact information for an alternate may also be provided.
Summary of Project: Indicate Educational or Research
Provide a brief synopsis of the project and indicate whether it is educational, research, or both.
Number of Participants in Activity
Provide the number of participants involved in the project. If this includes Penn faculty, please include faculty members' names. If participants are from outside of Penn, please provide an explanation of where these participants are coming from and in what capacity they are participating.
Exact Location of Activity
Obtaining space for this project is the responsibility of the Project Leader. Space must be identified and scheduled prior to submitting a proposal; all rooms used by the project must be approved by the Operational Committee. If you wish to use the gross anatomy labs, Stemmler 202 and 206, please contact Dwayne Hallman (hallmanj@pennmedicine.upenn.edu) to discuss their availability. Rooms that have not previously been approved for these activities will need to be inspected by the Operational Committee and approved prior to the onset of the activity. DO NOT SUBMIT THE CADAVER AND BODY PARTS REQUEST FORM UNTIL YOU HAVE IDENTIFIED AND SCHEDULED A ROOM.

This refers to when the activity is anticipated to begin and to end. Please be as accurate as possible.
Quantity and Condition of Cadaver(s) and/or Body Part(s) Needed
Please be Specific (e.g. 1 FRESH/FROZEN CADAVER, 2 EMBALMED/PRESERVED SHOULDERS, ETC.)
It is very important to be as specific as possible in this category. If you require fixed tissue, DO NOT SUBMIT THE CADAVER AND BODY PARTS REQUEST FORM UNTIL YOU HAVE CONTACTED the Morgue Manager, Dwayne Hallman (215-898-8054; hallmanj@pennmedicine.upenn.edu) to ensure that the fixed cadavers or body parts can be obtained and that they can be available at the requested time. If you are requesting fresh tissue, do not contact Mr. Hallman. See the Approved Vendors list for sources of fresh tissue. Also note that the use of non-fixed human tissue requires special precautions during the procedures as well as special cleaning of the labs afterwards; all this is the responsibility of the Project Leader. Source of Cadaver(s) and/or Body Part(s)
This will be from either existing inventory in the morgue of the SOM (through the Humanity Gifts Registry) or from an outside vendor, approved by the Committee regulating the use of cadavers and recognizable body parts for education and research at Penn. Cadavers/body parts may not be obtained at will, and no cadavers or body parts can be obtained from non-approved outside sources. If the SOM morgue cannot provide the requested material please reference this list of approved vendors. Also note that the used of non-fixed human tissue requires special precautions during the procedures as well as special cleaning of the labs afterwards; all this is the responsibility of the Project Leader. Please contact EHRS (215-898-4453; ehrs@ehrs.upenn.edu) for the SOPs for fresh tissue. AGAIN, IF FRESH TISSUE IS BEING REQUESTED, DO NOT SUBMIT THE CADAVER AND BODY PARTS REQUEST FORM UNTIL YOU HAVE DISCUSSED THE PROCESS WITH EHRS.
Exact Storage Location of Cadaveric Materials/Final Disposition
It is crucial that the Operational Committee know the location of the cadaveric materials at all times. Therefore, it is of the utmost importance that this information is known and/or updated whenever location changes occur. Final disposition refers to how the cadaveric material is disposed of when its use has been concluded: whether it is returned to the morgue, returned to the vendor, etc. There will be a charge for dispositions through the SOM morgue.
