Formal Complaint Form

You have the right to share any concerns or complaints about your experiences with the Cohen Clinic and receive a timely response. 

For minor concerns, we encourage you to submit feedback using our "Your Voice Matters" page, or discuss the issue with your provider or with the Clinic Director. If you are not comfortable discussing your concerns with your provider or the Clinic Director, or if you feel they cannot adequately address your concerns, then you may use this form to make a formal complaint to clinic leadership.
