Sending Records to ACE-WS

In order to meet each patient's individual needs and ensure that your visit goes smoothly, it is very important that we have all records at the time of our team's review and prior to any scheduling. There are many ways to send your records:

  1. Upload the records to myCHOP or myPennMedicine - 
  2. Send the records to our email
  3. By Fax at 215-701-0824
  4. Mail to:

If this is your first time visiting our clinic, please send us your records via email or fax. 

If you are seeing specialists outside of our core multidisciplinary clinic team (Lead WS physician, physical therapy, occupational therapy, cardiology, and psychology), please contact each department at least one week prior to your appointment to ensure they have all necessary medical records. Since we may not be familiar with the specific requirements of each department, it’s crucial to verify that they have all relevant information before your visit.


Requesting Your CHOP/Penn Medicine Records

To obtain your records from CHOP or Penn Medicine, please refer to the links below:

Accessing Medical Records via the MyPennMedicine Portal

If you would like to access your adult family member's medical records through the MyPennMedicine portal, please complete the Proxy Authorization Form. Once completed, a chart will be created for you, and a sign-up link will be sent via text or email to help you complete the account setup and gain secure access to your family member's health information.


Form Requests

For forms that need to be completed by our clinical team (e.g., guardianship, school documents, clearance paperwork, etc.), please upload them to the myCHOP portal and notify our team via a myCHOP message. Kindly allow up to two weeks for our clinical team to process and complete these forms. Please note that processing time may vary depending on the complexity and volume of the paperwork.
