Insurance and Billing
Our team assists with appointment scheduling and coordination of visits. It is important to note that different providers at CHOP and PENN have unique insurance coverage restrictions. We recommend you consult with your insurance carrier to ensure that the visit(s) will be covered and you are aware of any out-of-pocket costs you may incur.
If you require referrals through your insurance, you must have them prior to your appointment being scheduled (click here for a detailed list of referral requirements, including CPT codes and provider NPIs). Without the appropriate referral, your visit may need to be rescheduled, or you may have to sign a waiver and agree to be billed for the full cost of your visit. For CHOP patients: If your insurance is non-participating (non-par), we are unable to schedule your appointment until we receive out-of-network authorization. To facilitate this process, please have your primary care provider submit a referral for each specialist and send it via email (aceforws@pennmedicine.upenn.edu) or fax (215- 701-0824) for our records. Please note that obtaining out-of-network authorization can take 4-6 weeks.
