*If a candidate has limited teaching experience and/or a letter of assessment is not available from the candidate’s home institution, a statement of explanation must be included in it's place.
* A resignation letter is needed when candidates are transitioning from the Standing Faculty (Clinician Educator & Tenure tracks) to an appointment as an Assistant Professor in the Academic Clinician track. Department Faculty Coordinators must attach this resignation letter and candidate’s CV to the Termination AA Form and submit to the Office of Academic Affairs.

*Departments may be required to submit an Education Portfolio for candidates currently working in non-faculty roles at Penn or CHOP affiliated practice. Contact OAA to determine if this will be necessary.
